I am also immensely grateful to Associate Professor Dolezhal, he directly operated on me and subsequently observed the recovery process
Specialist Aleksey helped me very carefully, personally and for sure. Dad successfully underwent prostatectomy in the Czech Republic. I thank all the people involved in organizing Bookimed !!! Testimonial from patient Sergey: Dear visitors of the site "Bookimed", hello. I want to share my story with you. My name is Sergey, St. Petersburg, I am 65 years old and I have had prostate cancer. 03/19. 19 I got a PSA 6.26 analysis result. (I periodically donated blood for PSA, as there were some problems of a urological nature, besides, I was at risk, my father had prostate cancer) I turned to an oncologist, did an MRI of the pelvis, which showed that I in the left lobe of the gland, a capsule of 20x8 mm, conclusion: PIRADS 5, as bad as possible, as the doctor said. Next, the biopsy that confirmed the cancer, “Acinar Adenocarcinoma of the Prostate,” the total score for Gleason 6. Of course it was a hit! Although I understood that this is quite possible. My relatives helped me a lot: wife, children (I have two sons), wife of the eldest son (I do not like the word daughter-in-law). And then the question, "where to do the operation?" His son was told about the existence of the Bookimed company, which helps people in finding the best treatment option for various diseases around the world. He turned to this company, where he was immediately answered and offered two, the most acceptable options, the Czech Republic or Germany, the operation on the Da Vinci robot. Frankly, I was inclined to have surgery in St. Petersburg (three clinics have Da Vinci robots), but after talking with our doctors and the organizer of the operation at St. Zdislava Hospital in the Czech Republic, Irina Korchminskaya, I came to the conclusion that the operation should be done in Czech Republic !!! About a second, later did not regret it! As soon as I agreed to the operation, (previously I sent the results of the examination), on May 17, I was immediately scheduled for surgery on June 3. They immediately told me that the operation will be done by an assistant professor Dolezhal (experience, more than 2000 operations on prostate cancer on a robot !!!), assisted by Professor Tvaruzhek. All information about the clinic and doctors in the public domain on the website of the hospital of St. Zdislava. This is one of the many reasons why I had surgery there, and not in Russia, I could not get information on the statistics of operations performed by doctors in St. Petersburg. I flew to Prague 05/01/19, I was met at the airport and taken to the hospital, there I was met by the translator Otakar Peshek, speaks excellent Russian. 05/03/19 I had surgery, everything went well. I want to thank the entire staff of the hospital, all the great professionals, have been working in this hospital for many years, starting from nurses. Of course, many thanks to Professor Tvarujek, he is a great professional and a wonderful person! For all my busyness, whenever I had questions, I found time to answer them and give recommendations. I am also immensely grateful to Associate Professor Dolezhal, he directly operated on me and subsequently observed the recovery process. Special thanks to Dr. Khamzin! He did not participate in the operation and was not my attending physician. He graduated from the Leningrad Medical Institute, but for about twenty years he has been living and working in the Czech Republic, he stopped by several times, asked how he was doing and made recommendations for further recovery after being discharged from the hospital (gymnastics, etc.). While in the hospital, I contacted the employees of Bookimed three times. , on issues that have arisen with me and now, two months after the operation, I keep in touch with them, as well as with the St. Zdislava hospital, thank you all very much! Now everything is fine with me, almost completely recovered. 08/01/19 donated blood for PSA, the result is 0.026. I want to tell everyone who is faced with the same problem as mine, do not despair! Cancer, this is just a diagnosis! He can be defeated! You just need to turn to the professionals on time! And in the hospital of St. Zdislava wonderful doctors and nurses work! During my stay in the hospital, people underwent surgery: from Latvia, Belarus, Azerbaijan, Croatia, Mexico, Russia !! While in the hospital, I met one person from Riga, with the same problem as mine. We arrived with him on the same day, we performed operations with him on the same day, and on the same day we headed home. While walking in the surroundings (after the operation you need to walk as much as possible, and the hospital is located in the reserve!), We had the idea to travel across Europe from the hospital of St. Zdislava to the Spanish city of Santiago de Compostela on kickbike scooters under the slogan "we beat cancer!". We want to help people who for the first time encounter the same problem as ours understand that cancer can be defeated! We know how important support and faith are for such people, we have come this way! Perhaps this run will draw someone's attention to the existence of this problem and it will reveal this disease in time! The sooner a person turns to specialists with this problem, the easier it will be to win! If someone has questions, I will try to answer everyone, write Read more less

"He is very happy that he finally got rid of the cancer using the da Vinci robot
The Medipol Clinic in Istanbul was recommended to me by bookimed specialists. I was very pleased with the additional service. We were met with my father at the airport and taken to the guest house, which was provided by the hospital free of charge, as well as free 3 meals a day, not only for my father, but also for me. Every day a car came for us to take us to the clinic, as well as to take us back. We were provided with the services of an interpreter. Translator Murat turned out to be a very good person, he told everything and helped absolutely everything. operas atsiyu. Everything went fine, he is very happy that he finally got rid of the cancer using the da Vinci robot. The staff after the operation behaved very courteously and neatly. Read more less

Bookimed is a company of a good modern level, in any case do not think not to trust
I want to evaluate the work of Bookimed as a whole on the results of organizing the treatment of a father well. After examining the results of the surveys, we were presented with options for treatment. Despite the fact that initially we practically had a clinic for treatment selected (on our own), we revised and made a decision to undergo treatment at a clinic in Germany, which Bookimed offered. For all organizational issues, both Bookimed and the logistics company from the clinic offered support. We did everything on our own, but still it is nice when they want to help you. There were minor inconsistencies in organizational issues. But with the mines that provide support, always in touch and help them solve. But as for the treatment process at Helios Krefeld, everything is on the level. The result is satisfied. Of the minuses - the discharge process is not fully organized, the time for receipt of documents is not specific and rather long. A translator is needed. After we returned from treatment, we are awaiting the receipt of documents (conclusions) from the clinic, we have not left us on this issue either. Bookimed is a company of a good modern level, in any case do not think not to trust. The main thing is to keep the level and only raise it. Read more less

For a long time I have chosen a clinic for performing robotic nerve-sparing surgery according to the Da Vinci method. In Kazakhstan, such operations are not performed. I wanted to go to Germany or Israel, negotiated with them, but due to covid restrictions I had to choose Turkey. And I did not regret it at all. From the proposed options I chose the Anadolu clinic. After 6 days I was already in Istanbul. The entire organization and coordination of treatment was at the highest level. For which I am very grateful to coordinator Eliza. She was always in touch and could be contacted for any question. Special thanks to the translators Zahida and Muhammad. They were very attentive and responsive, they were always interested in my condition and were in touch. Now directly about the treatment. After examination by an oncourologist and providing him with all the examinations at home, in 2 days I was scheduled for an operation. No extra examinations and money spinning were appointed. My attending physician Ilker Tinay performed the operation brilliantly. The operation lasted for about 5 hours, under general anesthesia, but by the evening I was already forced to walk, accompanied by nurses. And after another 2 days I was able to check out. In the department, everyone from the doctor to the orderlies performed their work competently and professionally. I would also like to say about the conditions. A separate room of about 20 sq. m with a shower, toilet, pull-out chair for an accompanying person, TV, refrigerator and even a safe. (My hotel room was worse). After I was discharged, the doctor examined me three more times and gave me recommendations. And now after 2 weeks I feel almost completely healthy and there are no side effects (although according to statistics this is possible and quite often). I would like to thank my attending physician Ilker Tinay for his qualified and attentive approach to my treatment. As a result, the cost of examination and treatment was significantly different for the better from the same in Europe or Israel. Thanks to everyone who took part in this.

I had a terrible experience. Dr. Zielinski was dismissive and rushed me, he hasn't answered my questions and he asked me for more tests in order to speak about treatment plans when I was told previously that I didn't need any tests in order to proceed with the consultation and receive a treatment plan. He didn't bother to explain the disease or treatment options. I was looking forward to speak to him and I put so much hope. I needed someone to explain and guide me. I was left with such a bed feeling after the consultation. No empathy and no professionalism. Bookimed apologise but they said that they can't do anything because I paid directly the clinic, even if they are the intermediary they don't take responsibility, so be very careful when booking any services. I wasted 400 euros for 10 minutes that I was trying to make a few questions in order to get some value for the money but the only thing I got was disappointment, frustration, stress and time waste for arguing. I asked for at least to have some questions asked in writing so I could get something back for my money, but they refused, and they said that the consultation was compleated. In the first place, they said that I had a second consultation for free because it wasn't compleated, and then they said it was a mistake and the consultation was compleated. Please be careful where you spend your money and put your hopes because they don't care about taking advantage of ill people in order to make money. Probably are not all the same but my experience was awful and I am very disappointed. Some become doctors because they want to help people and others have no feelings or empathy. I hope this helps whoever is reading and I hope you find a better option than I did with a doctor that cares and will help you. God bless.

Memorial is more than a hospital. It is home from home. Very hospitable and caring staff who show a lot of empathy. My one night stay in the hospital was a delightful experience, kind and available staff, every neat and hygienic room. The doctors are on top of their job. I was taken through a lecture in nuclear physics 101, broken in simple language. The lecturer helped allayed my initial fears about the treatment. The treatment was speedy and the results came so quickly. The doctor was by my bedside to explain the next steps. The translator was fantastic. The transportation service was superb, always on time. I got more than value for my money. I will strongly recommend Memorial Sisli Hospital for any person seeking Lu-177 therapy

Good afternoon. In October 2019, my dad (who needed to undergo a Pat-CT examination with gallium 68 (psma) visited the Medipol University Hospital. The impressions are different from the coordinators and the hospital itself. Everything is very coordinated and thought out. The coordinators "led" us starting from Ukraine and ending with serving in Ukraine. So I really want to thank Anna, Olga, Zulfiya, Elmira, Babadzhan and others for their attention and support. At the hospital we underwent a qualified examination, consultation (though we had to pass additional tests, which were are necessary for the doctor) and the recommendations of the necessary doctors (the translator Elmira was always there, so we understood everything). Everything was at a very high level. The next day, after the examination, we received all the documents in Turkish and within a week they sent me translations in English and Russian languages by e-mail. After 3 months, my dad and I plan to fly for re-examination. I really hope that this time the team of coordinators will also work 100%! Guys! Thank you very much!

Very quick and clear consultation of Bookimed coordinators. For several hours, they helped organize a consultation at the clinic and lure the flight. A transfer from the airport to the city was also organized. In the clinic, with the help of coordinators, a consultation with explanations was successfully held, diagnostics were carried out, and the treatment process started. Very good attitude of doctors.

In principle, the visit went very well, only good impressions remained. Now there are certain problems, because doctors at home do not agree with the diagnosis that was made in China, but we were provided with professional help at the Food Clinic. Despite the fact that the full-time translator was on vacation and we took it on our own, the visit went very well. The rooms are very good, they look like 5 star rooms. As soon as we arrived, the nurses came to the ward, measured the pressure, helped in everything. In general, the clinic staff is friendly and helpful.

My name is Sergey, I am undergoing treatment at the oncology department of the Nordwest Clinic in Frankfurt am Main. I would like to note the high professional level of doctors, the technical equipment of the clinic at the proper level. I want to express special gratitude to Professor Elka Yeager, senior doctor Kiselichki, Dr. Weber, the highest level of professionalism, are very friendly and attentive, the junior staff are smart, everything is super, I’m very pleased with the treatment, if not for one “but” ... Because of ignorance language had to contact the company "Phoenix Service", which is an intermediary for the provision of medical services, led by Radislav. My communication with them began with the fact that before arrival they demanded a significant prepayment, which was timely transferred to their account. After arriving at the clinic, I had to transfer money several times, rather big money. I persistently asked the company’s employees to conclude an agreement with me on the provision of services by Phoenix Service, in order to specifically understand which services and for what specific payment the services will be provided. However, no contracts, as well as invoices with prices, were provided to me, which directly testifies to dishonesty. Only two months later I received the clinic’s bills (with which I completely agree) and separately a bill from Phoenix Service for the services rendered, after viewing which there were simply no words ... As it turned out, being treated in Germany was not expensive, it was expensive to pay for the services of Phoenix invoiced, which either didn’t turn out at all, or turned out to be in the wrong amount and at very high prices. I tried to talk with the head of the company Radislav, saying that I did not agree with the bill, to which I received the answer "if something does not suit you, you can refuse our services further." I officially wrote an appeal to the clinic about the detachment from this dishonest company, to which I received a positive response. He also wrote an official letter to Phoenix with a request to review the account, to which he still has not received an answer. I want to warn everyone who is in my position, those cancer patients who sell everything in order to pay for treatment and live on, but these “phoenixes” don’t stop at anything to enrich themselves at their expense.

When they learned what was needed in the clinic, they didn’t even know where to run and whom to contact. Coordinators helped to select a clinic. They gave us a doctor. Everything went great, the doctor is a true professional.  Separately, I want to note the attitude to patients. We were met at the airport, brought to the clinic and there was already a translator waiting. Literally "by the hand" we were taken to the doctors, everyone translated. Then they provided a taxi to the hotel.   During the operation, the ward was provided for two people - for the patient and the attendant; both of them were included in the cost of the operation.   After surgery, the doctor held a consultation, making sure that you can fly back. And only then answered questions remotely that arose.  Clinic definitely recommend

The tumor is removed with a laparoscope through small incisions - up to 2 cm. The procedure saves the patient’s erectile and urinary functions.

Radiotherapy is conducted by Teknon specialists who use Novalis linear accelerator. It determines the size of the tumor and selects the dose of irradiation. The device accurately affects the tumor and does not affect the surrounding tissue.

Chemotherapy is prescribed for men with metastatic cancer.

The choice of treatment option depends on the type of tumor, age and general condition of a patient.

My dad, radiation therapy, 40 sessions, about 2 months of stay in Istanbul were treated in the clinic. It's still early to speak about the results, therefore the review is exclusively about the process (!) For now. The choice helped to make bookimed, and I highly recommend using this excellent service. As a user, I would like to name the most basic plus. First and foremost, it is time saving, and in some cases it is equivalent to the preservation of health and life. And what is important, without costs for the patient, there is simply no payment for the company's service. Moreover, service coordinators are a huge resource for patients. We had Konstantin, and we are very grateful for the help and the choice of the best medical solution (at the price / quality / reputation of the doctor / clinic) and great help in solving logistical issues. When we have to deal with "medical tourism", there are no tourism patients for the patients there: (In addition to the medical problem, there are many practical, financial, coordination issues. It is important to have support on the side of the patient, we are very grateful. Now about the clinic: in general, the clinic liked, very modern and good equipment (so it was chosen), a good price-quality ratio. . From the pros: the doctor radiologist and the staff of the radiological department were attentive and in matters of medicine, and just a relationship. 40 sessions are 40 visits to the clinic, it is difficult, both physically and morally. The benevolent staff really helped make this experience less painful in all respects. I will not write much about the interior of the clinic, but for whom the aesthetics are important, yes, everything is excellent, modern, clean, often more like a hotel. As elsewhere, there is a large team of translators. To communicate with doctors used English directly, there certainly are many advantages. But all 40 sessions and not only were always assisted by translators in communicating with junior honey. personnel, coordination issues, medical records to the doctor, analyzes, transfers, etc. From the minuses: The clinic is in a very expensive and difficult physically accessible area (on a hill). Despite the fact that we lived side by side (significantly overpaying compared to other areas of Istanbul), we had to use motor transport every day. For a short stay, this is not a problem, but when a longer period is still an inconvenience. Some translators of the clinic introduced the category of "minuses" of the clinic. People are different, qualification, too, but it was still in a significant minority (our main interpreter was both professional and human). Again in the minority (!), But for the objectivity to mention, some doctors offered super expensive tests in very standard situations. For example, oh, well, if you want, in addition to the X-ray, we can do another CT scan (when coughing), which besides doubtful honey. values (not to mention money) was recently and so done. Again, our main doctor is very intelligible and enthusiastically explained the essence of the treatment, the expected effect, side effects, prescribed drugs on time to eliminate them, which of course, most important! As a conclusion, the minuses will always and everywhere, but in general, from our experience, we are satisfied with the process of treatment in the LIV hospital. Thank you hospital and bukimed team.

High qualification, operational work at any time of the day, sincerity - only a small description of her qualities. Special thanks for providing meals to the patient and accompanying person. FAQ about Prostate cancer Treatment

What is Prostate Cancer?

Prostate cancer is a type of cancer that occurs in the prostate gland, a small walnut-sized gland in men that produces semen. Since 2014, the incidence rate of prostate cancer has increased by 3% each year in the U.S. In 2024, about 299,010 new patients have confirmed the unpleasant diagnosis.

What is the Function of Prostate Gland?

The prostate is a part of the male ctive system. It is located beneath the bladder and in front of the rectum. It surrounds the urethra, the tube that carries urine out of the body.

The seminal vesicles are two minor glands attached to the back of the prostate. Both the prostate and seminal vesicles produce fluid that mixes with sperm to create semen. During ejaculation, this semen travels through the urethra and exits the penis.

Who is in the Risk Group of Developing Prostate Cancer?

Prostate cancer is very common in men over 65 years old. The studies show that about 1 in 8 men will get diagnosed with prostate cancer during their lifetime. The severeness of the disease and symptoms depend on several factors:

  • 👨🏻‍🦳 Age. Prostate cancer doesn't happen much in men under 40. The chance of early-stage prostate cancer increases after 50 years old. About 6 in 10 prostate cancers are found in men who are 65 years or older.

  • 👨🏾‍🦱 Ethnicity. Prostate cancer is more common in African American men and Caribbean men of African descent. In these groups, it often happens at a younger age. On the contrary, prostate cancer occurs less frequently in Asian American, Hispanic, and Latino men. The reasons for these racial and ethnic differences remain unclear.

  • 🫂 Family history. Having a father or brother with prostate cancer more than doubles a man's risk of developing the disease. The chances increase significantly if multiple relatives have had prostate cancer, especially if they were diagnosed at a younger age.

  • 🧬 Gene mutations. In a small number of cases, specific gene mutations are among the risk factors. For example, variants of the BRCA1 or BRCA2 genes, which are associated with higher risks of breast, ovarian, and other cancers in some families, can also elevate prostate cancer risk in men, particularly mutations in BRCA2.

What are Prostate Cancer PSA Levels?

PSA stands for "Prostate Specific Antigen". It is a protein produced by normal and cancer cells in the prostate gland. Higher blood levels of PSA can be the first signs of prostate cancer.

There's no specific data on what level of PSA is normal. However, the practitioners consider PSA levels of 4.0 ng/mL and lower acceptable. Men with a PSA level between 4 and 10 (often called the “borderline range”) have about a 1 in 4 chance of having prostate cancer.

Besides the growing cancer cells, PSA and prostate cancer risks depend on:

  • Age;

  • Prostate gland size;

  • Level of inflammation;

  • Ejaculation;

  • Vigorous exercises (such as cycling) 2 days before the exam;

  • Drugs, such as finasteride and dutasteride, are prescribed for treating benign prostatic hyperplasia (BPH).

PSA screening is mandatory for all men over 50 to prevent prostate cancer. The doctors often use it together with the Digital Rectum Exam (DRE) for patients reporting prostate symptoms. However, the PSA test is not a "golden standard" for diagnosing prostate cancer. It indicates several benign (not cancerous) conditions:

What is the Gleason Score for Evaluating Prostate Cancer?

When prostate cancer is detected in biopsy samples, a pathologist assesses how aggressive it might be by giving it a grade. One of the most popular evaluation tools is the Gleason system. It measures how quickly the cancer cells are likely to grow and spread.

The Gleason system, which has been used for many years, grades prostate cancer from 1 to 5 based on how closely it resembles normal tissue. A grade of 1 means the cancer looks a lot like the prostate gland, while a grade of 5 indicates that the tissue appears very abnormal. Grades 2 through 4 represent a range between these two extremes.

The Gleason score for prostate cancer combines the two most common grades in the biopsy samples. The first number represents the tumor. For example, if the score is 3+4=7, it means most of the cancer is grade 3, with some grade 4, and these numbers are added together to get a total score of 7.

That's why men with a Gleason score of 3+4=7 cancer tend to do better than those with 4+3=7 prostate cancer.

What are the Stages of Tumor Growth?

The Tumor, Nodes, and Metastasis (TNM) system determines how far prostate cancer has spread in the body. It helps doctors understand the stage of the tumor.

Prostate cancer that is confined to the prostate is generally easier to treat successfully. However, if the cancer has spread (metastasized) outside the prostate, it usually requires treatments that affect the whole body, such as drug-based therapies.

  • 🩻 T (Tumor) score indicates the size and extent of the original tumor in the prostate;

  • 🩻 N (Nodes) score shows whether the cancer has spread to nearby lymph nodes;

  • 🩻 M (Metastasis) score reveals if the cancer has spread to other body parts.

Prostate cancer has four main stages:

  • 🟢 Early-stage | Stages I-II. The so-called "localized" cancer because the tumor doesn't spread beyond the prostate. The doctors can't see the cancer during the first stages with imaging tests.

  • 🟡 Locally advanced | Stage III. Cancer cells spread outside the prostate but only to nearby tissues. The doctors detect tumors during DRE or transrectal ultrasound;

  • 🔴 Advanced | Stage IV. The cancer spreads to lymph nodes, bones, liver, or lungs.

Some stages of prostate cancer split further (for example, IIA, IIB, IIC, etc.). Within each stage, an earlier letter signifies a lower stage. Although every individual's cancer experience is unique, cancers with similar stages often have a similar outlook and are typically treated in a similar way.

What is the Grading for Prostate Cancer?

The doctors divide prostate cancer patients into 5 groups, according to the Cambridge Prognostic Group (CPG). The risk group indicates how likely cancer cells will grow and spread.

The Cambridge Prognostic Group (CPG) is a system doctors use to predict the aggressiveness of prostate cancer. It classifies cancer into one of five groups (CPG 1 to CPG 5) based on such factors:

  1. PSA levels;

  2. Gleason score;

  3. Metastasis.

CPG 1 indicates a lower likelihood of rapid growth or spread, while CPG 5 signifies a more aggressive cancer with a higher chance of spreading. This classification helps doctors develop the most effective prostate cancer treatment.

Group CPG 1 CPG 2 CPG 3 CPG 4 CPG 5
PSA result ≤ 10 ng/ml 10-20 ng/ml 10-20 ng/ml ≥ 20 ng/ml ≥ 20 ng/ml
Gleason score 6 3+4=7 4+3=7 8 9-10
Tumor stage 1 or 2 1 or 2 1 or 2 3 3

What are the “Red Flag” Symptoms?

Early-stage prostate cancer doesn't show many symptoms. However, as the cancer cells grow, the following signs may appear:

  • Frequent urination, especially at night;

  • Difficulty starting or stopping urination;

  • Weak or interrupted urine flow;

  • Pain or discomfort in the lower back, hips, or pelvic area;

  • Blood in the urine or semen;

  • Erectile dysfunction;

  • Unexplained weight loss or fatigue;

  • Loss of appetite;

  • Bone pain.

Other diseases can cause these symptoms as well. However, as prostate cancer remains the second most common cancer in men, its early detection can make a big difference in successful treatment outcomes.

What are the Screening Methods for Prostate Cancer?

Besides the PSA test, there's a whole range of exams to treat prostate cancer successfully.

Digital Rectal Exam

During a digital rectal exam (DRE), the doctor inserts a gloved, lubricated finger into the rectum to check the prostate for any unusual bumps or problematic areas that could be signs of cancer.

Because the gland is in front of the rectum, the doctor can feel bumps or solid areas in the back in case of prostate cancer.

The procedure might feel uncomfortable, especially if there are swollen veins, but it typically isn’t painful and only takes a few moments.

MRI or Transrectal Ultrasound (TRUS)

Also called a prostate sonogram or endorectal ultrasound, this test is often combined with a prostate biopsy. It allows to visualize the prostate gland and surrounding tissues. The TRUS-guided prostate biopsy has shown positive results in 14-27% of men who had negative biopsy tests before.

Prostate Biopsy

A core needle biopsy is one of the main diagnostic tools for prostate cancer. It is the choice for patients who have confirmed their prostate cancer with PSA and DRE tests.

To diagnose prostate cancer, doctors take tissue samples either through the wall of the rectum (a transrectal biopsy) or through the skin between the scrotum and anus (a transperineal biopsy). The urologists take a few tissue samples to examine them through a microscope.

Although the procedure might seem painful, it causes only a short period of discomfort. This is because a special tool quickly inserts and removes the needle. Most specialists will use a local numbing medication before starting the biopsy to make the area less sensitive.

The prostate biopsy takes about 10 minutes and is often performed in the doctor’s office. The doctors often recommend taking antibiotics a few days before the procedure to prevent infection.

What are the Main Treatment Options?

Treatment for prostate cancer depends on several factors, including how fast the tumor is growing, whether it has spread, and the patient's overall health.

For a man diagnosed with localized or locally advanced prostate cancer, there are three main treatment options:

🔬 Active Surveillance

This method implies monitoring prostate cancer closely, without radical solutions. It includes regular doctor visits with revised PSA and DRE exams. The specialist might suggest other treatment options if the cancer changes its dynamic. This approach is best when the prostate cancer doesn't cause any visible changes or symptoms, such as trouble urination, and is expected to grow slowly;

👩🏻‍⚕️ Surgery

The primary surgery for prostate cancer is called prostatectomy. It involves removing the prostate gland, surrounding tissue, and a few lymph nodes.

There are a few ways to access the prostate gland:

Surgery is an option for treating cancer that’s confined to the prostate and may be used to treat advanced prostate cancer in combination with other treatments.

⚡️Radiation Therapy

Radiation therapy uses high-powered energy to kill cancer cells. Prostate cancer radiation therapy treatments may involve external beam radiation therapy or
brachytherapy (internal radiation), which uses a machine to direct high-powered energy to prostate cancer.

Radiation therapy might be considered for treating prostate cancer, depending on its stage and other factors:

  • As the first treatment for prostate cancer that is still within the prostate gland, especially if it's in a lower-risk category. In these cases, the success rates are similar to those of men who undergo radical prostatectomy (92.4% for radiation therapy compared to 96.1% for surgery);

  • As part of the initial treatment, combined with hormone therapy, for cancers that are still in the prostate but are higher-risk or for cancers that have spread to nearby tissues;

  • If surgery doesn’t altogether remove the prostate cancer or if the cancer returns in the area of the prostate after surgery;

  • If the cancer is advanced, such as spreading to the bones, help control it for as long as possible and relieve symptoms.

💊 Hormone Therapy

Prostate cancer cells rely on testosterone to help them grow, and cutting off the supply of testosterone may kill cancer cells or reduce their growth.

Hormone therapy for prostate cancer works by blocking the sex hormones called androgens (mainly testosterone), which cancer needs to grow. This can be done in a few different ways:

  • Reducing the production of androgens by the testicles;

  • Blocking the effects of androgens throughout the body;

  • Stopping the body from making androgens altogether.

🩸 Chemotherapy

Chemotherapy is not a standard treatment for prostate cancer patients. Chemotherapy-exposed men have shown higher survival rates (76.3%) compared to chemotherapy-naive men who didn't receive treatment (survival rate 70.5%).

Chemotherapy can be used with hormone therapy or by itself. It is an option for advanced cases, mainly when the cancer has spread extensively beyond the prostate gland.

🩺 Immunotherapy

Immunotherapy helps the body’s immune system kill prostate cancer cells. One type of immunotherapy approved by the FDA is a vaccine called sipuleucel-T (Provenge). It increases the 36-month survival probability to 31.7% from 23.0%.

Sipuleucel-T is different from traditional vaccines, which are designed to prevent infections. Instead, this vaccine boosts the immune system to help it target and attack prostate cancer cells.

This vaccine works for advanced prostate cancer that no longer responds to hormone therapy and is causing few or no symptoms.

The doctors prescribe Sipuleucel-T individually. The medical personnel extracts white blood cells from the patient's blood to produce a vaccine.

What are the Specific Prostate Cancer Treatment Options?

Method of treatment Brachytherapy (Radiation Seed Implants) Proton Therapy for prostate cancer HIFU (prostate ultrasound) Leuprolide injection Cryotherapy
Who’s it for? Men with early-stage prostate cancer who are in a lower-risk group. When the cancer has spread to the lymph nodes. If the cancer comes back after radiation therapy. Sometimes for early-stage prostate cancers. For treating advanced prostate cancer. If the cancer comes back after radiation therapy. Also an option for men with low risk who can’t have surgery.
Survival rates 99.3% 94.4% 90.1% 87% 93.9%
Main advantages Shorter treatment time (10-20 minutes per session for high-dose therapy). Lower radiation dose to surrounding tissues. Cancer destruction in the targeted area without radiation or incisions. Relieves painful/difficult urination. Less blood loss, shorter hospital stay (usually one night), and recovery period.
Main disadvantages It can cause incontinence or pain during urination. The method is more niche and expensive. Narrowing of the prostatic urethra (stenosis). Increased risk of heart attack and stroke. Erectile dysfunction is more common in contrast to radical prostatectomy.

How to Pick the Best Hospitals for Prostate Cancer Treatment?

The right hospital and well-qualified doctors are the main factors for successful treatment outcomes. Here’s a quick guide on finding the best prostate cancer treatment centers.

  • Research Hospital Reputation. Hospitals known for excellence in prostate cancer care should be a top priority. Global rankings and reviews can give some valuable insights;

  • Check Accreditations. The accreditations from international health organizations ensure quality diagnostics and treatment;

  • Doctor's Expertise. The clinics with experienced prostate cancer specialists, especially those engaged in prostate cancer research, count;

  • Treatment Options. The ideal options should have access to the latest technologies (such as robotic surgery, HIFU, and brachytherapy) and personalized care plans;

  • Patient Reviews. Testimonials from patients with high prostate cancer levels are insightful to assess the care quality.

Is it better to have the prostate removed or treated?

Both treatments are effective, and the risk of cancer spreading is low with either option.

However, each prostate cancer treatment comes with side effects, including potential issues with the bladder, bowel, and erections. Radiation therapy is more likely to cause bowel problems. 10-20% of men report having diarrhea a few times each week after the procedure.

What is pT3b Prostate Cancer Prognosis?

pT3b means a locally advanced prostate cancer that has spread into seminal vesicles on both sides. It is a high-risk disease associated with a negative outcome. The overall survival rates for this stage are 59-84% in a 10-year period.

Nevertheless, patients with clinically significant prostate cancer still have treatment options.

These include:

  • External beam radiation therapy (EBRT);
  • Hormone therapy;
  • Radical prostatectomy.

What is the newest and best treatment for prostate cancer?

In 2023, the FDA approved a medication called enzalutamide. It can be used alone or with another drug called leuprolide for men who have a rise in PSA levels (biochemical recurrence) and are at high risk of their cancer spreading.

Do patients need a urinary catheter after prostate removal?

During a radical prostatectomy, the surgeon inserts a catheter into a penis to help drain the bladder. The catheter has to stay in place for 1 to 2 weeks during healing. In a robot-assisted prostatectomy, though, the catheter doesn't stay longer than 2 days. After the catheter is removed, normal urination can resume.

Can a patient ejaculate after prostatectomy?

After surgery, the feeling of orgasm should still be enjoyable, but there won't be any ejaculation of semen—this is known as a "dry" orgasm. This happens because the doctor removes the prostate and seminal vesicles, which produce most of the fluid for semen, and cuts the pathways for sperm.

For some men recovering from prostate cancer, orgasms may feel less intense, and in rare cases, some might experience pain during orgasm.

Does Viagra work after prostate removal?

After surgical prostate cancer treatment, Viagra can help with erectile dysfunction, especially if the procedure did not significantly affect the nerves. However, everyone responds differently, so finding the correct dose might take some time.

Can a man with no prostate get a woman pregnant?

During a radical prostatectomy, the surgeon cuts the vas deferens, which are the pathways that carry sperm from the testicles to the urethra. Although the testicles will still produce sperm, the sperm can no longer leave the body as part of the ejaculate, meaning that natural conception is no longer possible.

Men with prostate cancer are often older, but for those who still wish to have children, it's possible to preserve . Before surgery, they can store sperm in a sperm bank, allowing the option for fatherhood through assisted ctive methods later on.
